How can I tell if she’s faking an orgasm?

It’s hard to tell. Some signs of an orgasm include increased vocal expression and muscle tension (she moans loudly and arches her back, for example), a flush of her skin or a release of tension, followed by a feeling of deep relaxation. On the other hand, she might experience one or several of these things and not “feel” she’s had an orgasm. However, other than just focusing on her orgasm, it’s also important to find other ways of getting her to feel good.
